How to make an appointment

    • To make an appointment for a New Patient the referral letter must be sent in to the Out-Patients Department, the Consultant will review the letter and an appointment will be sent out as soon as possible to the patient.

 

    • The referral letter must preferably be typed and include, the patients name, full address, date of birth, gender, and a description of the child's complaint.

 

    • When making an appointment or inquiring about an appointment please be ready to quote the patient's full name, address, date of birth and medical record number if the child has been to the hospital before.

 

    • A return appointment can be made by, visiting the appointments desk or telephoning the Out-Patients Department.

 

    • If it has been a long time since you made the appointment or you have moved address you should ring to check that the appointment still stands before you arrive at the hospital.

 

    • It is very important that you ring and cancel your appointment if you no longer need it or cannot make the date given as in most cases another follow-up appointment is sent to the patient and if you miss 2 appointments you will be discharged back to your original source of referral.
